Insurance Verification & Payment
- Insurance is verified at every visit or test. Please bring your insurance card to each appointment.
- Copays and deductible amounts are due at the time of service.
While we verify Eligibility & Benefits for your insurance, it remains your responsibility to understand what copays and deductibles apply to your plan. If you are unsure, please contact your insurance provider using the member services number on the back of your insurance card.
